Create your account, build your first form and start collecting responses.
Survtapp lets you build surveys, checklists and waivers, collect responses online or fully offline, and act on the results in real time.
Your first form in three steps
- From the dashboard, click New Form and choose a type: Survey, Checklist or Waiver.
- Add questions with the drag-and-drop builder, then set your branding, logic and settings.
- Publish and share the form via link, QR code, email or embed — or collect on a device in kiosk mode.
Responses appear instantly under Responses and Reports, with charts, exports and a map of collection locations.
