Form Types: Survey, Checklist & Waiver

What each of the three form types is for, and when to use which.

Every form in Survtapp is one of three types. The type is chosen when you create the form and determines which fields, defaults and reports make sense for it.

Survey

Purpose: gather structured feedback or data from respondents — customers, event attendees, employees.

  • Common uses: customer satisfaction (CSAT/NPS), post-event feedback, market research, employee pulse surveys, lead-retrieval forms.
  • Key features: all 20+ question types, conditional/chain/skip logic, welcome and thank-you screens, page breaks, per-question and cumulative charts (bar/pie/NPS), a world map of collection locations, and CSV/Excel/PDF/JSON export.
  • Respondents can usually be anonymous; contact fields are optional and configurable per form.

Checklist

Purpose: a recurring, structured task list completed by staff or agents — inspections, audits, site visits, opening/closing procedures.

  • Common uses: facility inspections, safety audits, equipment checks, daily opening/closing routines.
  • Key features: recurring scheduling (Checklist Schedule + Checklist Instance) so the same checklist is auto-assigned on a cadence, location capture, barcode/QR scanning for asset checks, and kiosk mode for shared-device collection.
  • Checklists are typically completed by logged-in Staff/Agents rather than the public, and are tracked per instance (due, completed, overdue).

Waiver

Purpose: collect a legally-signed consent or liability release, with a durable record of who signed and when.

  • Common uses: activity liability waivers, photo/media release forms, terms acceptance.
  • Key features: a required signature question, contact fields enabled by default (name/email are needed to send the confirmation), consent text and version tracking, and an automatic confirmation email with a PDF copy of the signed waiver (including IP address, approximate location and submission time).
  • Anonymous responses are disabled by default since a waiver must be attributable to a named signer.
