FITNESS ACTIVITY WAIVER AND RELEASE OF LIABILITY
I, the undersigned, desire to participate in fitness activities offered by [Facility Name]. I understand that participating in fitness activities involves certain inherent risks, including but not limited to: muscle strains, sprains, fractures, heart attacks, and other injuries.
I represent that I am physically fit and have no medical condition that would prevent my participation in fitness activities. I agree to consult with a physician before beginning any exercise program.
I HEREBY RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE [Facility Name], its officers, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me while participating in fitness activities.
I have read this agreement, fully understand its terms, and sign it voluntarily.
