COVID-19 ASSUMPTION OF RISK, WAIVER, AND RELEASE OF LIABILITY
I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I (and, where applicable, my family) may be exposed to or infected by COVID-19 by participating in the activities and programs offered by [Company Name], and that such exposure or infection may result in personal injury, illness, permanent disability, or death.
I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury or illness that I may experience.
I hereby release, covenant not to sue, discharge, and hold harmless [Company Name], its employees, agents, and representatives, from any and all liability, claims, or demands of whatever kind arising out of or relating to exposure or infection by COVID-19, to the fullest extent permitted by law.
